The confusion about the bone hourglass is that keepsakes do not automatically equip, you have to manually switch them on before runs or after bosses at the big display looking thing you can interact with, so it was not having an effect right away when you got it

Next weapon you get should be the bident it’s next to the sword cause it has a lot of reach and does good damage. The shield on enemies acts as a second health bar and also prevents them from getting stunned from your attacks.
